Rates and Insurance
I am currentlly an in-network provider with Blue Cross Blue Shield, Cigna, MHN, MHNet, United Behavioral Health and Value Options. I am in process of transferring provider status from Madison, WI to Dallas, TX for other insurances. I would encourage you to check with your insurance company to see if I am on your plan. I also accept cash-pay clients. Contact me and we can discuss rates privately.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the 50% of your fee as booked.
