Insurance
Currently, I am an in-network provider for The Alliance, MuliPlan EOS, WEA Trust, Tricare, Aetna, United Behavioral Health, MH Net, Value Options and Cigna. I am registered as an out-of-network provider for Blue Cross Blue Shield. There are other smaller insurance companies I contract with, as well.
I will help you check with your insurance provider to determine if you have out-of-network benefits. If so, I can provide you a receipt for services and you may be reimbursed through your provider.
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the 50% of your fee as booked.
